Thanks to @heartukcharity for invitation to deliver plenary lecture. Our UK CPRD data show that hyperTG is not a uniform CV risk factor but impact varies across vascular beds and patient populations, highlighting the need for precise cardiovascular risk stratification.

New data presented by Dr Bilal Bashir explored the relationship between hypertriglyceridaemia and cardiovascular risk using the UK CPRD database.
Key takeaways:
• Hypertriglyceridaemia was associated with increased cardiovascular risk, but the pattern differed between primary and secondary prevention.
• Risk also varied across different vascular territories, highlighting that triglyceride-related risk is not uniform.
• The findings support the need for population-specific and vascular territory-specific risk assessment, moving beyond LDL-C alone.
An interesting reminder that addressing residual cardiovascular risk remains an important challenge.

Be PRAGMATIC
Be SIMPLE
From Haffner to VESALIUS: we didn’t discover anything new… we finally accepted it.
📍 1888, Haffner (NEJM) already warned us:
👉 A patient with diabetes and no MI carries a risk similar to someone who already had one.
nejm.org/doi/full/10.1056/NE…
📍 2026 VESALIUS (JAMA) just removes the last excuse:
👉 Even before clinically evident atherosclerosis, the risk is already biologically active.
jamanetwork.com/journals/jam…
👆 Let’s be honest:
There are NO LOW-RISK patients with diabetes
There are NO INTERMEDIATE-RISK patients with diabetes
👉 There are only:
HIGH RISK
VERY HIGH RISK (most of them, if you actually look closely)
📌 The problem is not classification.
📌 The problem is clinical denial and therapeutic inertia.
We don’t undertreat diabetes because we lack evidence.
👉 We undertreat it because we are still thinking in outdated risk categories.
Atherosclerosis starts early.
Diabetes accelerates it.
And we… keep labeling instead of acting.

The genetic architecture of FCS is more complex and heterogeneous in the UK:
- 12.9% of the tested are genetically confirmed FCS
- Over half are LPL-FCS, but non-LPL FCS are equally common
- Regional & ethnic variability
