Sharing our latest in @AmJClinOncol!
Does how far a patient lives from a cancer center influence whether they receive radiation—and what type?
We studied this in people with WHO grade 2–3 meningioma.
Proud of @gorantla_nipun for leading this work!

Our commentary @IJROBP on virtual care in #radonc in response to @CShahMD's thoughtful piece.
A few points to emphasize. A brief 🧵
First, it is important to define the regulatory landscape and separate two concepts that are too often collapsed: supervised technical services versus the radiation oncologist’s personal performance of professional services.
Radoncs supervise the services delivered by RTTs. When we talk about supervision, this is the type of activity that is being supervised.
In contrast, evaluating, adjusting, and signing CBCTs prior to SBRT, on-treatment management, plan evaluations, etc are the professional services radoncs perform.
An important threshold question: does federal law permit virtual supervision and virtual performance of professional activities?

V different diseases, but working with @imrtlee taught me that maybe if good response we don’t need the full dose escalation🤔if no GTV after neoadj ADT —> is 36 Gy enough? If doesn’t respond —> is 42 (eg should we push 45+)?

The @FDA recently approved a targeted therapy called lirafugratinib (Lyrfigtu) for people with advanced cholangiocarcinoma (bile duct cancer) that has a genetic change called an FGFR2 fusion or an FGFR2 rearrangement.
The clinical trial that resulted in the drug’s approval was led by MSK early drug development specialist Dr. @AliSchram.
“Cholangiocarcinoma is a rare cancer that is almost always caught after it has spread, making it extremely challenging to treat,” Dr. Schram says. “Any time we find a new treatment for patients with this disease, it can make a meaningful difference to them and their families.”

Based only on these 3 slides :), it looks like no difference in tox but, at least for HPV negative dz, you have significantly more failures in the weekly Cis regimen. The CI for the LRF difference between the two regimens in the HPV negative population doesn't cross 0. Reinforces our local practice of q 3 weekly Cis. Maybe I'm missing something....

Seems like there is a notable separation early on. Still need to follow up longer for both p16+ and p16- groups to see durable LRF results.

Fame is how widely you are known (can be known for good or bad things). Legacy is what remains because you were here.
Fame is largely about attention in the present. It can come from achievement, celebrity, visibility, wealth, or simply being associated with a major event. It is often measured by how many people know your name?
Legacy is about enduring impact. It asks: What did you change? Who did you influence? What institutions, ideas, discoveries, people, or values continued after you? A person can have enormous legacy without ever being particularly famous.
Always chase impact. Change the way the world does things whether they know your name or not.
Pick any medical specialty other than your own. Who’s the most famous physician in it? You probably can’t name one.
Almost none of us will be known outside our field, and even those who achieve real professional fame are mostly forgotten within a few decades.
Fame is a poor reason to devote your life to medicine. The work itself matters more: caring for patients, discovering new knowledge, and teaching the next generation.
