Could some cases of migraine be on the same disease continuum as idiopathic intracranial hypertension? Interesting proposition! academic.oup.com/braincomms/…
Another therapy for thyroid eye disease - satralizumab (IL-6 inhibitor)!
SatraGo trials: pubmed.ncbi.nlm.nih.gov/4270…
Ginger powder has a similar efficacy to sumatriptan for acute migraine... Really makes me wonder how much we actually know about migraine.

Interesting study suggesting that in patients on DOACs, there is a increased signal for levetiracetam having a higher thromboembolic risk compared to other ASMs.

Agree completely. While I am amazed by and appreciative of the science behind them, I am highly skeptical that these drugs are the answer to Alzheimer disease.
Hell of a business model. Anti- amyloid drugs in asymptomatic elderly people first trials
Specious end points to trials

Exciting results, CAR-T may be the future in many neurological diseases!
Among 16 patients with refractory neurologic autoimmune disorders, lentiviral CD19 CAR T-cell therapy was associated with manageable side effects, complete B-cell depletion, and preliminary clinical improvement across disease groups. Full trial results: nej.md/4zPPwbD

ALT A figure showing the clinical outcome measures after treatment, including scores on the Expanded Disability Status Scale (EDSS; range, 0 to 10, with higher scores indicating greater disability) in patients with MS or MOGAD (left), Manual Muscle Testing 8 (MMT-8) scores (range, 0 to 150, with higher scores indicating stronger muscles) in those with IIM or gMG (middle), and scores on the Quantitative Myasthenia Gravis (QMG) scale (range, 0 to 39; higher scores indicate greater disease activity) in those with gMG (right).
The concept of autoimmune B12 central deficiency (ABCD) is so interesting. May account for many cases of idiopathic myelopathy, looking the same as subacute combined degeneration of the cord with dorsolateral cord changes.
Among patients with idiopathic #Myelopathy, screening for anti-CD320 antibodies with metabolic confirmation of central nervous system–restricted #VitaminB12 deficiency may help clarify diagnosis and guide treatment evaluation.

New treatment for thyroid eye disease: veligrotug, a full antagonist of IGF-1R. Different to teprotumumab, which is a partial antagonist.

Dysthyroid optic neuropathy, optic neuropathy occurring in the setting of thyroid eye disease, characteristically affects the inferior visual field and often have a predictable pattern of progression

Excellent overview and update on blunt cerebrovascular injury (BCVI) by @drcraighacking in #Radiopaedia2026, including updates to the Biffl grading scale tiered by degree of stenosis, and what the natural history looks like and how 90 days may be a useful time point for imaging.
Important point from @daniel_gewolb in #Radiopaedia2026 regarding the false localising value of a CN VI palsy - can occur if the brainstem is shifted due to either intracranial hypERtension or hypOtension.
Good article on false localising signs: jnnp.bmj.com/content/jnnp/74…
Day 1 #Radiopaedia2026 great overview slide from @francisdeng of the many faces of sarcoid myelitis.
New to me was the anterior myelitis pattern mimicking compressive myelopathy, accounting for 10% of cases.
Read more about this pattern: neurology.org/doi/10.1212/NX…
Great pearl from @daniel_gewolb on Day 1 #Radiopaedia2026: "ordinary" progressive multifocal leukoencephalopathy (PML) does not have mass effect or enhancement. If it does, then consider PML-IRIS, which can occur at the same time as initial PML or later as worsening.
Interesting work on interleukin-6 receptor blockers (tocilizumab or satralizumab) being used in acute management, within 1 month of clinical attack nadir, in MOGAD and NMOSD. Promising addition to the acute armamentarium?

It is not just sellar masses (e.g. pituitary macroadenomas) that can cause a bitemporal hemianopia... Ethambutol-induced optic neuropathy is not one to miss.

⚠️Breaking news⚠️: pyridostigmine helps myasthenia gravis...
Newsflash: A randomized controlled trial supports a treatment that millions of patients been using over the last 70+ years!
This could have gone two ways, and speaking on behalf of everyone who cares about MG care, all I can say is... Phew!
pubmed.ncbi.nlm.nih.gov/4194…
...and amifampridine does not help myasthenia gravis

Settles the debate following earlier conclusions drawn from registry data.

OVERLORD-MS: Anti-CD20 monoclonal antibodies are effective in relapsing multiple sclerosis. However, data from trials directly comparing different agents are lacking. Research findings from a phase 3 trial of rituximab vs. ocrelizumab are summarized in a new Quick Take video. nej.md/4y3DFG8
Newly proposed entity: seronegative MOG antibody syndrome (SMOGAS). This would include seronegative steroid-responsive entities such as chronic relapsing inflammatory optic neuropathy (CRION) and acute disseminated encephalomyelitis (ADEM).

Rare sporting event supporting a neurological condition, so had to go. RIP Neale, your contribution to MND funding in Aus has been profound. #BigFreeze12 (hope the Pies lose!)
