I'm not anti AI (I use it every day)...but since I'm bedside with patients daily, I'll share how SOME patients are using LLMs and what I'm seeing with one visit to my office this past week. This is why health care costs will probably go up short term before AI can bring them down. Patient has labs come back with an ALT (liver enzymes) 5 points above upper limit of normal at his annual physical. His doc sends him a message most likely from overweight, to lose some weight. The patient then goes online and an LLM tells them they might have livery injury, might be best to get labs and agrees with their doctor it might be due to weight. Patient goes to another primary care office after his doctor refused to order a bunch of labs via just a portal message (looks like he requested a follow up visit if he wants to talk about them in much more detail beyond his message) and pt decides to talk to the new primary care doctor and the following was ordered after he tells an LLM recommended: CMP GGT Hepatitis profile Iron profile + Ferritin ceruloplasmin ANA Antismooth muscle antibody INR/PT/PTT Then when all came back negative with same 5ish point elevation, the LLM program told him he needs an ultrasound. So back to his second primary care office with this where the doc does another visit and agrees to order it. The ultrasound shows fatty infiltration. So now the LLM convinces him he needs a fibro scan which came back similar, with mild fatty liver disease He can't get into the second primary care doctor so comes to see me (third primary care office) where I reaffirm that yup...hes 100lbs overweight....nothing else compares to weight loss and we have a discussion about all the options. When people are saying AI is going to decrease costs...help patients and doctors....this is just one example of a visit I had this past week with this type of patient who will go down the health anxiety rabbit hole. This guy spent probably over $1,000 of his health insurance dollars on the additional workup and at least 3 extra doctor office visits. Currently no LLM program is (IMO) willing to say something like, yes you are 100lbs overweight, strong probability it's just like your doctor said. Lose weight and let's recheck in the future and no additional labs needed at this time based on lack of symptoms or risk factors. BCBS just for this one case probably spent over $1,500 on his care because he went down the LLM rabbit hole of what it might be.

This is a tough post for me to make. Even though I have been disabled for six years, in my mind I believe I should be independent and able to handle my own finances. It is a tough pill to swallow that the expenses which come with being quadriplegic and ventilator-dependent are too enormous to manage with my own resources. I have lived an incredibly simple life - I own two pairs of shoes, a few shirts and pants, and the basic essentials. Several of you have encouraged me to create a fundraiser for myself, but I prefer to spend money on those who are truly in need...and I struggle to wrap my mind around the fact that I am in need. Being blindsided by the State of Minnesota's incompetence and corruption has changed my family's situation. My wife contacted Ramsey County last week regarding their illegal termination of my benefits, but as of today they have not reinstated anything...they say they're "looking into it." They have zero concern that I have lost my critical prescriptions, respiratory supplies, and home health care aides. They do not care that they have endangered my life. While it is exceedingly embarrassing for me, we have created a fundraiser to help me get back to Neuralink for my required follow-up visit. Realistically, at $38K, we will not be able to make this trip more than once, so we have to consider a permanent move to Florida to remain in the Clinical Trial. As explained in a previous post, I am not allowed to fly commercially, due to my ventilator. Attempts to drive 36 hours without my complete ventilator heater setup have landed me in the ER, and my Pulmonologist will not give me medical clearance to try that again. My wife sought four estimates from air ambulance companies with the capacity to fly ventilated patients. The quoted prices are $38K, $42K, $50K, and $62K (yes, my wife asked that last company if this was round-trip, and they said it is only one-way 😯). We would like to accept the $38K quote if we can raise the money. Please excuse my wife's too-kind narrative about me in the fundraiser summary. I also want to be transparent that the first $500 we raise, if we raise that much, will probably go to my respiratory Rx and supplies for this week, but the remainder will go to the air ambulance expense. I will try to have an update for you later this week. On Thursday, I will be doing my very first interview since my Neuralink procedure (actually, very first since my accident). As a Tesla fanboy, I am happy that this interview will be hosted by @teslaownersSV who have done an amazing job of learning my story. If you have the time, please join us. If you have questions, post them here and I will try to address them. And as for the fundraiser...it is here: givesendgo.com/air-ambulance…

Congrats Maple! I’m so happy this is finally out. Over the last few months we worked with @TryMapleAI on a complete overhaul of their new visual identity, a Maple Research redesign, and a full design of their new Maple Agent (coming soon). Maple is known for being the most private AI. So we wanted to create something that felt really solid and trustworthy but still approachable and soulful. I’m so honored you chose us for this project, it was so awesome working with you guys. @anthonyronning @marksuman If you’re tired of letting Sam and Dario read all your ai chats, you have to try Maple → trymaple.ai

OpenAI’s lead attorney William Savitt just gave a press statement outside the trial. His frame: Shivon’s testimony confirmed there was no pre-existing deal with Musk to keep OpenAI a nonprofit. Savitt arguing her testimony directly contradicts what Musk testified to last week.

Caught Marc Toberoff (Musk’s lead attorney) on video giving a press statement outside the OpenAI trial. His frame: the nonprofit has been diminished, OpenAI is clearly a massive for-profit. This is what Musk’s closing will sound like.

He kept going by thinking of his family, God, friends and Thomas the Tank Engine.
Incredible story of courage and faith from Western Australia. 13-year-old Austin Appelbee, swam for four grueling hours through massive waves and cold, dangerous waters to save his mom Joanne and siblings Beau (12) and Grace (8). Swept out to sea while kayaking and paddleboarding near Quindalup, the family drifted up to 14 km offshore. His mom made the heartbreaking call to send him for help, he ditched his life jacket to swim freer, prayed nonstop, and promised God he'd get baptised if he survived. "I don't think it was actually me... It was God the whole time," he said. After collapsing on shore, he sprinted ~2 km to alert authorities, leading to a helicopter rescue that night. What a remarkable kid, officials called his effort "superhuman." 🙏💪

It's crazy that the worst part of a post condoning murder is that he has the economics backwards (premiums would go up if fewer claims were denied)

Cash pay marketplaces like @mishehealth are the future because they connect cash pay patients with cash pay providers. You cut out the rent-seeking health insurance cartel. you save big $$ by paying cash direct. I just got an ortho visit, consult, and two cortisone shots for $250. Mishe found me the doctor after my own searches yielded doctors who didn't even understand I was paying cash. they wanted $1000 for a ten minute job. Mishe fixes that.
