The trio is finally reunited! 🎉
What a pleasure to catch up with my great friends and colleagues @VivekSubbiah @Timothee_MD at #ESMO2026 in Madrid!
And what better occasion to celebrate our latest publication in the Journal of Clinical Oncology! 🍾
ascopubs.org/doi/10.1200/JC…
Great science, stimulating discussions, friendship, and a shared passion for challenging established thinking and improving cancer care.
Because science is even better when you get to share the journey with people you genuinely enjoy working with!
Here’s to collaboration, and many more papers together! 🥂

What a nice evening in Madrid.
Great conversations with inspiring colleagues!
These moments make conferences special!
With Emanuela Romano, @ProfCUBlankNKI, @Rikke_Andersen_, Inge Mansfield Noringriis, @DoniaMarco, Inge Marie Svane & @EvaEllebaek.
Hey #ESMO26 !
Come to see our 2nd poster, today, 12 am, Hall5!
We often talk about informative censoring for drugs, but what about informative censoring in non-drug oncology trials?
Like any intervention, exercise, behavioral, and supportive care interventions can be burdensome for some patients, potentially leading to dropout.
Our work suggest some level of "burden"-driven informative censoring may happen in such trials + see our sensitivity analyses incl. the CHALLENGE trial!👇
Good morning #ESMO26
Later today, 12 am, come to see our poster, Hall5!
A "mixed" approval cover adult and paediatric population, but how many children are actually enrolled in such trials?
12% overall representation (see Iceberg plots!)
FDA grant more mixed approvals, younger ages
See more details and our conclusions 👇
Nice to see the MASTERPLAN study presented #ESMO26 and published in @TheLancetOncol !
Congrats to the authors!
Our new paper :
"A new era in anti-RAS therapy: Unique challenges and opportunities for radiotherapy clinical trials"
with @free_radical28 is out in EJC, open access :
sciencedirect.com/science/ar…
Food for thoughts for ongoing and future trials in that space!👇
Encouraging pancreas SBRT data from MASTERPLAN. Adding 40 Gy/5 fx cut 12mo locoregional recurrence from 52% to 20% (P=.022).
OS 22 vs 14mo, but this is a 74-pt phase II with mixed resectability. Promising; not definitive OS evidence.

NATALEE 6-y results are out! #ESMO26
2.1 delta in survival at 6 years.
Thanks @Annals_Oncology for reporting subsequent therapy data!
👉Only 44.4% of control arm patients with distant recurrence received a first-line CDK4/6i
Does this survival difference truly inform clinical practice in settings where CDK4/6i are standard of care for metastatic disease?
In my opinion, the answer is no.
#ESMO26
NATALEE 6-y OS results are online, ahead of today's presentation (2:25 pm).
OS HR 0.809 (95% CI 0.661–0.989); 6-y OS: 91.9% vs 89.7% (Δ2.1%). Nominal 1-sided P=.019.
What remains key? 👉 Post-protocol care!

🚀 ESMO Congress 2026 ,Here we go!
The wait is over! #ESMO26 officially kicks off today!
I’m delighted and honoured to be one of the #ESMOAmbassadors for one of the most exciting and influential events in oncology worldwide.
Over the next few days, science will take centre stage, bringing together the global oncology community to share groundbreaking research, challenge current standards, and shape the future of cancer care.
🔬 Will we see practice-changing data? I suspect we will ,and I can’t wait to discover, discuss, and critically appraise the results!
📢 Stay tuned! I’ll be sharing highlights, key findings, and my perspectives on the most exciting developments throughout the congress.
Let’s follow the science, challenge the evidence, and keep pushing the boundaries of cancer care.
Let the science begin! 🔬🌍

HR 0.58 is right where the phase 2b was, and they used a real pembro control, which I give them credit for.
Unfortunately this is RFS on top of a pembro backbone that was itself approved on RFS. COMBI-AD showed big adjuvant RFS and DMFS gains with no significant OS difference.
Patients who recur now have good salvage options, so treating earlier isn't necessarily curing more people.
Fever in 52% vs 8% means nobody was really blinded. I'd want OS before calling this a new standard.
These results with adjuvant Moderna personalized vaccine added to pembro for melanoma look good to me, and are a positive indicator for the adjuvant and perioperative lung cancer trials. #LCSM
#ESMO26 tomorrow the 6-y survival results of the NATALEE trial will be presented in Madrid, 2:25 pm
trial = 3 years adjuvant ribociclib in patients with HR+ HER2- early breast cancer.
what will be key? 👉 Post-protocol data

PATINA Trial👉Dr. Olivier called it in January. In October, the EMA agreed.
Jan 31: "makes PFS results less reliable, particularly in open-label trial" — @Timothee_MD on #PATINA
Jun 24: @FDA approval.
Oct 16: EMA says the benefits "had not been demonstrated." @pfizer

🚨⭐️⭐️⭐️Just Published 👉Right in Time for @myESMO #ESMO26👉 Pleased to share our new paper in JCO @JCO_ASCO @ASCO
👉⭐️ “Time-of-Day Immunotherapy: Why the Field Still Needs Randomized Evidence.”
⭐️⭐️⭐️OPEN ACESS+ FREE for all to read & comment
⭐️👉@Timothee_MD , @Alfdoc2 & I set out to compare our views on RCTs. We AGREED on one point immediately 👉 THAT this question deserves a definitive answer
👉Retrospective data suggested that the time of day an immunotherapy infusion is given may affect outcomes. The first randomized phase III trial that appeared to confirm this has since been retracted, and the question is open again.
👉Our view: the optimal timing of immunotherapy remains unsettled, and robust randomized evidence is the only responsible way forward @OncoAlert @oncodaily ascopubs.org/doi/10.1200/JCO…

Love that three people who disagree on RCTs agreed on the question. The practical takeaway for fellows until the trial exists: don't delay or reschedule an infusion just to chase a morning slot.

"Statistics in Oncology in Practice", fom my talk available at theoncologyshot.com/p/my-fri…
A change in sample size should always be transparently explained.
If not, there is a risk of what we call "p-hacking", "data-dredging"
See here with an example with the PANOVA trial👇

🚨 PATINA: The EMA questions a 15-month PFS gain (HR 0.75). I highly recommend 📖theoncologyshot.com/p/the-pa… by @Timothee_MD:
An open-label trial, investigator-assessed PFS, and estimated early censoring of 12.1% in controls vs 5.3% with palbociclib.
Robust benefit or potential assessment bias?
Not surprised:
My take on PATINA:
theoncologyshot.com/p/the-pa…
EMA has raised concerned over Patina trial results . Company has withdrawn the application at EMA for said indication .EMA feels results may have been biased ?? @PTarantinoMD @dr_yakupergun @ErikaHamilton9 @hoperugo @drsarahsam

Great to share some thoughts on “Mentorship 101” at the #AILCC26 🇮🇪
One concept I wanted to highlight: hedonic adaptation or “becoming accustomed to success”
Yesterday’s dream becomes today’s normality and tomorrow’s expectations.
In medicine, we constantly chase the next publication, grant, promotion or achievement. But do we ever stop to appreciate how far we’ve come?
We guide a d help our mentees on how to be successful. How about guiding them on how to feel successful? @DrJNaidoo @FordePatrick @gerryhanna
