#ESMO26 @PTarantinoMD presents the novel and unique Charisma trial with matched samples starting an ADC then with PD. My take? Resistance is incredibly complex and not due to an isolated pathway. Down reg of receptor target uncommon. @OncoAlert

MOIO: Interim Analyses of a Phase III Randomized Non-Inferiority Trial Assessing Reduced Frequency Immune Checkpoint Inhibitors in Responding Patients With Advanced/Metastatic Cancer
@SabatierRenaud
🔬 Academic phase III non-inferiority trial (n=340, France): standard IO vs reduced dose intensity (every 3 months) in patients responding after 6 months of IO for advanced solid tumors.
➡️ PFS HR 1.35 (0.9–2.0); median 21.9 vs 25.0 mo; futility boundary exceeded
➡️ OS HR 2.0 (1.1–3.9) with reduced IO
➡️ Grade 3–4 irAEs 9.5% vs 10.5%
✅ Authors: non-inferiority not achieved, no meaningful safety advantage, and a signal of inferior survival; differential effects argue against uniform de-escalation.
