The ? begs for a cleaner RCT. LUCKILY @theusinghal and @swog have created S2613 which has the “do or don’t do question” built into cohort A and further explores the role in (mi)N+ disease on PSMA PET. Hopefully urologists are on board w/this trial and we start answering some ?s

Massey becomes one of two sites in VA to offer single-port robotic surgery for prostate cancer.

Please publish the series. Would be genuinely novel, supposedly with an enormous n. Would be nice to know which patients were treated, by what modality. Perhaps there is learning here, perhaps exaggeration. I believe the focal community would welcome the insight.
If I had a nickel for every time a friend asked me about prostate focal therapy, I’d be rich.
I’ve personally managed 20–30 intraprostatic failures after focal therapy. One patient ended up with a permanent Foley after salvage RT. Nearly all required medical castration, which many likely could have avoided with definitive therapy upfront. The downstream consequences are real.
Our group ultimately decided not to offer focal therapy outside of very niche scenarios (for example select reirradiation not ideal for partial or full gland SBRT). If I were a patient, I might assume focal therapy is better than standard options too. The marketing is that good. #OncTwitter

So much fun hosting @vourganti @VCUUrology! Powerhouse talks and amazing clinic teaching. Thrilled that such a great guy is pushing our field forward. Can’t wait to see what awesome innovations he comes up with next and for future collaboration! @FocalSociety
So grateful to join the dynamic and talented group at @VCUUrology today as Visiting Professor to their Dept. Many thanks go to my good friend @apkenigsberg and Chair Adam @klaussner for the generous opportunity to share ideas towards the future of prostate cancer precision care.

Two-handed endoscopic surgery is a game changer! No doubt the applications will extend beyond en bloc resections. Can’t wait until this is readily available and can hopefully bring to @VCUUrology
Hot off the press!! First reported case series of transurethral robotic en bloc resection of bladder tumour using the Virtuoso Endoscopy System (VES). Published in @EurUrolOncol today! (1/n) #ERBT #UroSoMe

Agree Andrew.
What people are failing to consider when looking at the data:
1) PSA is cheap, non-invasive and very sensitive
2) The decision to biopsy is different now than it was 23 years ago. We use PSA density, MRI, genetic tools
3) Utilization of Active surveillance is MUCH higher now. Cancer don’t equate to treatment
4) We start screening earlier and stop screening earlier now
5) we have better RT planning, higher utilization of robotics, developing focal therapies
6) people fail to realize that it wasn’t until about 15 years ago the we really started to develop an effective way to treat metastatic prostate cancer.
This same study conducted now would provide less harm and atleast similar benefit. But is it truely reasonable to conduct massive RCTs when the answer is right in front of us?
I would argue no. It is cost prohibitive.
PSA must have a role. It certainly needs to be better and more risk adjusted.
But those calling to stop all PSA are missing the point.

Exciting 23 year update from the ERSPC #prostatecancer screening trial from @MoniqueRoobol and team, and a great editorial @VickersBiostats. NND is down to 12 and still falling! And the benefit:harm ratio would be even better w/ modern screening/dx/tx pathways.
At 23 years of follow-up, population-based PSA screening of asymptomatic men led to a sustained reduction in prostate cancer mortality, although overdiagnosis remains a concern. Full ERSPC study results: nej.md/42YYsg7

President Biden’s diagnosis of metastatic #prostatecancer is serious—but in the past 20 years, 5-year survival has improved markedly thanks to better imaging, advanced therapies, and more effective care. Often men live many years—and sometimes decades—with a terrific quality of life.
Former President Biden has been diagnosed with an aggressive form of prostate cancer just months after his first term came to an end.
