Happy to share the final results of the 🇪🇸 & 🇫🇷 ph2 trial of intratumoral BO-112 + pembro for patients with anti PD-1 resistant #melanoma. 25% ORR and 54% patients alive at 2 years. 16 years of basic and clinical research behind. Some context here 👇🧵

Thank you @ivanpantic1980
I am very happy you enjoyed it. It was indeed a out of the box idea - showing how we can learn, and moreover do cross-fertilization from other tumor areas, and an academic trial! Congratulations for the great effort.

Novedades en melanoma del #ESMO26. Empezamos con STARBOARD (fase 3): encorafenib + binimetinib + pembrolizumab vs pembrolizumab en 1L BRAF V600. SLP 33,0 vs 3,4 m (HR 0,33), ORR 70% vs 32,7%. Pero: 70% TRAE G≥3, 42,7% suspende BRAF/MEKi y brazo control discutible.
My personal take on the (again) NADINA up date: we cannot omit adjuvant therapy in near pCR; only in pCR #melanoma #ESMO2026 @GrupoMelanoma . Before making decisions from asumptions from small trials, we should think about how we translate them into confirmatory trials.
This is a great achievement for our @GrupoMelanoma In around 6m, the group managed to include all the patients, with an out of the box idea in one single country. Brilliant presentation by @g_develasco Thanks to @myESMO for selecting it & @TeresaSAmaral for her great discussion
🚨 #ESMO26 | PLUG-IN GEM 2301
Enfortumab vedotin + pembro in anti–PD-1–refractory advanced melanoma (N=60):
▪️ Confirmed ORR 25% | DCR 55%
• BRAF WT: ORR 21.4%
• BRAF-mut: ORR 33.3%
• Acral 33.3% | Mucosal 20.0% confirmed ORR
Safety with no new signals!
Interestingly, Nectin-4 expression did not identify responders….exploratory, but worth watching 🤔
A new ADC signal in refractory melanoma…this PLUG-IN deserves to stay connected 🔌
Congrats to @g_develasco and all the @GrupoMelanoma team! 👏🏼

🚨 One of the big melanoma moments of #ESMO26!
INTerpath-001 is positive, but the most interesting part may be the questions it opens:
1. Who benefits most?
2. Will the RFS benefit translate into OS?
3. And where does a personalized mRNA vaccine fit in the neoadjuvant era?

Dr. Mario Balsa (@HospitalClínic Barcelona) discusses pembrolizumab + personalized mRNA vaccine V940 in resected melanoma.
📊 2-year RFS: 82% vs 60% (HR 0.58)
📊 DMFS: 88.5% vs 80.2% (HR 0.62)
Will this translate into longer survival? Who benefits most? And how does it fit with neoadjuvant immunotherapy?

👑 Presidential #ESMO26! INTerpath-001: personalized mRNA vaccination reaches phase III in melanoma.
Resected stage IIB–IV melanoma, N=1,137: intismeran + pembrolizumab vs pembrolizumab.
💥 At 24 months:
• RFS: 82.2% vs 70.0% → HR 0.58
• DMFS: 88.5% vs 80.2% → HR 0.62
• Absolute gains: +12.2% RFS / +8.3% DMFS
• Benefit emerges mainly after 6 months
• Grade ≥3 AEs: 23.6% vs 20.3%
• OS still immature
A clinically meaningful new mechanism, but the real question now is positioning 🤔
For macroscopic stage III: adjuvant personalized vaccine after surgery, or neoadjuvant ipi/nivo before it? Biomarkers may ultimately decide.
Congrats to @ProfGLongMIA and all the team! 👏🏼

These are the kind of reflections that appears after seing the data, and not based on speculations driven by casino-markets. Thank you for your sensible comment! @YannickBuccella
So the “not-so-official-anymore“ presentation is in: The melanoma vaccine works. We knew that, 82% recurrence-free at two years vs 70% with immunotherapy alone.
It's built individually for each patient, the logistics are enormous: Sequence their tumor, design the vaccine, manufacture it for one person. Weeks of work that will cost a fortune.
A treatment like that doesn't scale to most of the world. It scales to countries that can afford a factory run per patient. I live in a country which happens to be able to afford that, most countries can’t. Melanoma kills people everywhere. This will reach a fraction of them.
Solving cancer and solving access are two different problems. We just got better at the first one.

Some thoughts on the INTerpath-001 backlash, and the simplistic "neoadjuvant already does better, so why bother" take. I think it misses a few important things.
➡️ First, the neoadjuvant data (S1801, NADINA) come from patients with macroscopic, clinically detectable nodal disease. INTerpath-001 also includes node-negative IIB/IIC and patients whose nodal disease only showed up on sentinel node biopsy. In the big SLN trials, about two thirds of SLN+ patients had deposits ≤1 mm. None of these patients are neoadjuvant candidates. We see the same confusion in #NSCLC all the time, when people compare stage IIIA from adjuvant trials (pathological staging) with stage IIIA from neoadjuvant trials (clinical staging). Same stage on paper, not the same patients.
➡️ Second, INTerpath-001 was designed before NADINA read out, when adjuvant anti-PD-1 was still the default for resected stage III. Judging it against today's practice is a bit of hindsight.
➡️ Third, I suspect the most logical place for a neoantigen vaccine is after neoadjuvant ICI in patients who don't reach pCR. INTerpath-009 is already testing exactly this in NSCLC, so we'll get some hints there.
➡️ And all of this comes with only an absolute ~3% increase in grade ≥3 toxicity, which also belongs in the balance.
For macroscopic stage III, the critique lands. I want to see the full data and the stage breakdown at LBA1. But I'd be careful firing at this result before we've even seen it properly.

Lo que está pasando con la vacuna mRNA en #melanoma es un friendly reminder de que el mercado no entiende las cosas como creemos que las entendemos nosotros.
