Honored and proud to see our work sit in @NEJM! The patient is still in good health. Image of the week.

What is the convenience/cost/safety benefit of coronary CT angio based planning of PCI that would justify its use even if it resulted in 62,5% more target-vessel failures than regular PCI?

As TAVI expands to younger and healthier patients, valve durability has become an increasingly important consideration.
🎙️ In this #ACCEL Lite episode, Drs. @DLBHATTMD and @oyvlie examine the results of "ACASA-TAVI: Antithrombotic therapy after TAVI". ↪️ bit.ly/4AHtZ5w

🚨 ACASA-TAVI trial: DOAC monotherapy nearly halved subclinical leaflet thrombosis (HALT) after TAVI without increasing bleeding.
But HALT is a CT surrogate, the safety margin deserves scrutiny, and this study does not establish clinical benefit.
Here is what the result changes - and what it does not. 🧵 1/7

Dr. Øyvind Lie and Dr. C. Michael Gibson discuss the ACASA-TAVI trial: 12 months of NOAC monotherapy with FXa inhibitor reduced subclinical leaflet thrombosis and was non-inferior for bleeding, thromboembolic events, or death versus standard ASA 75 mg daily in TAVI patients aged 65-80. Watch video here: clinicaltrialresults.org/wp-…
Happy to discuss the findings and implications of @acasatavi with @CMichaelGibson! We are a little wiser, but much is still to be learned. @JAMA_current
ACASA-TAVI trial: NOAC monotherapy achieved non-inferiority for bleeding, stroke, MI, or death and reduced subclinical leaflet thrombosis compared to ASA 75 mg daily for TAVI patients aged 65-80. Download slides here: clinicaltrialresults.org/wp-…

After #TranscatheterAorticValveImplant, factor Xa inhibitor non–vitamin K antagonist oral anticoagulant monotherapy reduced hypoattenuated leaflet thickening vs acetylsalicylic acid monotherapy.

ARVC & exercise-induced arrhythmogenic #cardiomyopathy (EiAC) have similar presenting phenotypes, but only ARVC progressed over time. Despite the “morphological reserve” in EiAC, both had high rates VT/VF. jacc.org/doi/10.1016/j.jcmg.…

Both ACASA-TAVI and NOTION-4 are soon to be reported, should teach us a lot! @didier_tchetche @DFCapodanno

I personally still don’t know what to propose post TAVI. Apart from those with clear indications for DAPT or NOAC, I tend to stay with aspirin alone 🤷🏾♂️

RV structure/function deteriorated in #ARVC but remained stable in #EiAC.
Still, life-threatening VA were common in both.
✅ Stable morphology ≠ low risk.
👉 Highlights the need for proactive monitoring in athletes.

Inclusion in the ACASA-TAVI trial is closed! All participants have been randomized to NOAC or ASA following successful TAVI. We are one step closer to understanding optimal antithrombotic therapy after TAVI! @Oslounivsykehus @haukeland_no
