🚨Ronald Koeman reacts to Barcelona’s 2–1 El Clásico defeat to Real Madrid at the Camp Nou:
“You won’t go far in the Champions League with this backline.”
Look, I’m not here to sugar-coat anything. We’ve now had two proper tests this season — PSG and tonight against Real Madrid — and both times the same problems have shown up. We can control the ball, we can look good going forward, but the moment the opponent plays with real quality and intent, the spaces behind this defence are too easy to find. That’s not a coincidence anymore. That’s a pattern.
The high line is fine when the press works and the midfield is compact. Tonight it didn’t. Madrid broke it with simple vertical balls and quick combinations. The full-backs were caught high, the centre-backs were too far apart, and there was no recovery pace to cover the ground once the ball went in behind. Same thing happened against PSG. Different opponents, same picture. Top teams don’t need ten passes to hurt you when your defensive line is stretched and one or two players are already out of position.
Individual mistakes keep costing us as well. It’s not just the system. Players are losing concentration in the moments that matter. One bad decision, one late step, one failure to track a runner, and suddenly you’re 1-0 down in a game you were supposed to control. You cannot keep doing that against Madrid, against PSG, against the teams you have to beat if you want to go deep in Europe. Dominating possession means nothing if every time you lose the ball you look vulnerable. Attractive football in the first 20 minutes doesn’t help you when the decisive moments are decided by basic defensive errors.
I have to be honest: I’m not convinced this group of defenders is good enough for the level required in the Champions League. There are too many games where the same weaknesses appear. You need players who can defend one-v-one, recover quickly, and stay organised under pressure. Right now we’re relying on moments of individual brilliance at the other end to cover for structural problems at the back. That works against weaker sides. It won’t work when you face teams that punish every lapse.
The board has to stop pretending this isn’t an issue. Two clear warning signs already this season and we’re still talking as if the defence will just sort itself out. It won’t. You need proper reinforcements — players with the physical profile and the defensive reliability to handle this level. If they keep ignoring it, the Champions League campaign will end the same way these big games have: with the same problems exposed and the same excuses afterwards.
Tonight wasn’t an isolated bad night. It was another reminder. The evidence is there from PSG and from Madrid. If the club wants to challenge for the biggest trophy in Europe, they have to fix the defence properly and they have to do it quickly. Otherwise this will keep happening, and the results against the best teams will keep telling the same story.

🔥 #ESMO26 INTerpath-001, full data (now yes)
On top of RFS HR 0.58 and DMFS HR 0.62:
• Manufacturing success >99%
• >90% of patients got the maximum 34 neoantigens
🔍 Practice-changing? For a population, yes. Two things still to prove:
• Can an mRNA strategy rescue patients primary refractory to neoadjuvant ipi-nivo (no pathologic response)?
• Will it move OS in stage II-IIIB too?

🔥🎯 #ESMO26 NADINA, 3y update
Neoadjuvant ipi-nivo vs surgery + adjuvant nivo in macroscopic stage III #melanoma
➡️ 3y EFS 72.9% vs 50.0% | HR 0.42
➡️ 3y DMFS 75.3% vs 57.4% | HR 0.46
➡️ 3y RFS: pCR 91% | near-pCR 62% | pPR 88% | pNR 43%
➡️ G≥3 TRAEs 32% vs 16%
🔍 Read it next to the INTerpath-001 hype: in macroscopic stage III, adjuvant PD-1 alone is no longer the bar.
‼️: ~9% relapse after pCR, and near-pCR (no adjuvant) does worse than pPR (adjuvant given). Room for IO in 1-10% RVT?
🧬 RVT carries its own immune biology.

My personal take on the (again) NADINA up date: we cannot omit adjuvant therapy in near pCR; only in pCR #melanoma #ESMO2026 @GrupoMelanoma . Before making decisions from asumptions from small trials, we should think about how we translate them into confirmatory trials.

Superb presentation by Georgina Long! INTerpath-001: adding personalized mRNA vaccine intismeran to pembrolizumab reduced recurrence/death risk by 42% in resected stage IIB-IV melanoma (HR 0.58). A major step for cancer vaccines! #ESMO26 @OncoAlert @myESMO #ESMO26

#ESMO26 | INTerpath-001: My Personal Perspective Ahead of the Presidential Session
This innovation deserves recognition: a groundbreaking result for personalized cancer vaccines. However, in patients with macroscopic stage III melanoma who are eligible for neoadjuvant IO, this trial does not establish the added value over contemporary perioperative strategies. Neoadjuvant treatment can produce substantial responses while allowing some patients to avoid adjuvant therapy.
The trial design I would prioritize is:
Neoadjuvant IO → surgery → standard adjuvant therapy ± a personalized vaccine in patients without a major pathological response.
Here, “failure to achieve a major pathological response” is a more appropriate selection criterion than “any residual disease.” Patients with minimal residual viable tumor who still achieve a major pathological response should not automatically receive additional treatment.
This approach would direct treatment intensification toward patients at higher risk of recurrence. The key biological uncertainty, however, is that IO-resistant residual disease is not necessarily vaccine-sensitive. Whether vaccination can overcome that resistance needs to be demonstrated.
This criticism should remain specific to that population: INTerpath-001 retains distinct clinical value for patients with stage IIB/IIC disease and those who underwent surgery without neoadjuvant therapy.

Good news for #melanoma cancer patients: New results from a personalized #cancervaccine designed to prevent melanoma from returning after surgery showed positive results, according to research presented at this year’s #ESMO26 meeting in Madrid.
The study found that adding a specially tailored cancer vaccine to standard treatment with immunotherapy successfully reduced the risk of melanoma coming back by 42% compared with standard immunotherapy alone.
“The idea of this treatment is that by specifically boosting a patient’s immune system to recognize their own tumor cells, we can reduce the risk that the cancer comes back,” says Dr. Michael Postow, Chief of the Melanoma Service at MSK. “Even when we can’t see any cancer on a CT or PET scan, we know that often it’s still there — because such a high proportion of patients eventually have a cancer recurrence. That’s why there is tremendous need for this type of treatment.”

About the intismeran presentation at #ESMO26 $MRNA $MRK
My impression of discussant talk given by Inge Marie Svane of the University of Copenhagen was that she was positive if not overly enthusiastic about INTerpath-001.
"Measured" might be the best, single adjective.
"Brutal" is how an investor in the audience described the tone of her talk to me. (He was short Moderna, so there's that.) I disagree. Brutal is too harsh. If people were expecting Svane to jump for joy, then yes, she did not meet that expectation. To me, she played it mostly straight down the middle.
She took no position on the mRNA vaccine vs neoadjuvant debate. (Slide below)
She noted the lack of intismerna OS benefit, but said the same about neoadjuvant therapy. She also noted greater toxicity associated with neoadjuvant.
For intismeran, Svane also raised the question about what exactly was driving the vaccine’s benefit — was it the specific targeting of the neoantigens, or was it the general immune boost that studies have suggested mRNA vaccines generally provide?
